Completing the FSR form
- Select one option only: FSR on
an Existing Blanket, FSR for
a New Blanket, Request a Written Estimate (Fixed Price).
If you select
Existing Blanket Number, fill in the Blanket Order
Number and Tag Number (tag number is optional). Check the "Use Blanket as
box to have the original blanket information copied to this FSR.
Required fields are
marked with an asterisk (*).
- Enter the Department Code
directly or use the "Pick a Department"
link to look up a Department. Enter a full or partial Department Code
or Department Name and click "Search". Click on the Department
Code to return the information to the FSR. The Department information
is retrieved from a campus lookup file and cannot be changed from this
- Fill in the Name and Phone
Number of the Person to Contact in case
Facilities Management Customer Service or the shop involved has
questions about your service request.
- Fill in the contact person’s
email address. An email which acknowledges
receipt of the FSR will be returned to you. You do not have to have an
email address to submit this form to Customer Service.
- Fill in the Accounting Information only if you are requesting a new
Blanket Order or Written Estimate (Fixed Price Job). The Account(s)
will be verified against the Campus FAU List. Subs of (00, 01, 02, 06, and
will not be accepted. The Object is filled with "3175", but may
- Pick a Building from the
lookup list, and fill in the Room Number(s).
You need not repeat this information in the text of the Work Description
If you are requesting work in a corridor, patio or other common area,
describe the area and its location (e.g., place cork board in east
between rooms 31 and 33).
- Off-master defaults to
"No." If the room or area in which the work is to be
performed is off-master, select "Yes." Include information on
when access to the work area will be provided in the Description section.
- Alarmed defaults to "No." If the area is
alarmed either during regular or
after-hours, select "Yes." This tells us if work can be done
helps prevent the alarm from going off when we are there. If the alarm is
routinely turned off or can be turned off, please add this information to
text of the Description section.
- The Short Description should
be something that concisely identifies the job.
The Detailed Description field is where you can go into specifics
the work you wish performed. For example: The Short Description might be
“RENOVATE PROF. SMITH'S OFFICE”. The Detailed Description should
include all data on the work to be performed and when the work needs to be
scheduled. Examples of typical statements: "patch and paint faculty
April 7-14 when faculty member will not be here" or "add dedicated
circuit for additional copy machine scheduled to arrive on August 13, 1998."
Only use ASAP, URGENT or RUSH at the beginning of the Short Description
when the situation requires immediate fixing. An example of an "ASAP"
service request would "Broken prong stuck in electrical outlet.
“ASAP". You may include to whom questions should be addressed in the
Detailed Description section but not the telephone number or email address
unless it is different from that provided in the contact section.
- Check Associated Documents if
you are sending us such things as product
information or requirements, schematics, floor plans, renderings, etc. If
care to, let us know in the Detailed Description section that floor plans,
are being faxed or delivered. Our fax number is (310) 206-9461.
- Complete Authorized By, Title and Date Authorized (mandatory). If someone
else must authorize this FSR request, be certain that individual is the
who actually forwards it to Customer Service.
- For Key Orders Only assures
that the Hardware Shop has information on
where and how to get keys back to your department. Choose "Hold for
or "Mail to Above Address" depending on your requirements. If
keys are to be
delivered to someone or some address other than that provided at the top
the FSR, add that information to the Description section.
- When you have edited the FSR
and are certain it represents your needs
click on the "Submit" button. A message will come back to you
describing which fields have not been filled out properly or accepting
FSR and giving you a reference number. You may print out a copy of your
request. Please use your reference number when contacting Customer
Service. The FSR will typically be in our maintenance system in a matter
of minutes. You will receive an e-mail acknowledging receipt of the FSR by
Facilities Management (if an email address was provided).
